HealthLink’s My Aged Care e-Referral solution, powered by SmartForms, helps healthcare professionals complete referrals directly within their Practice Management System (PMS), reducing manual administration and helping patients access aged care assessment services sooner.

Designed around the way practices work

The My Aged Care e-Referral SmartForm has been designed to fit naturally into existing clinical workflows.

For users of supported PMSs, the SmartForm is available directly from the HealthLink homepage within the PMS. Practices not using a supported PMS can submit referrals securely through the MyHealthLink Portal.
