The online Mobility Parking Scheme SmartForm is intuitive, quick to complete, and accessible through HealthLink.

What are the new online Mobility Parking Scheme (MPS) medical certificates and how do they work?

HealthLink and Transport for NSW are introducing a new online Mobility Parking Scheme (MPS) medical certificate.

From 17 August 2026, medical practitioners will be able to complete and submit MPS medical certificates online through HealthLink. This digital process is designed to streamline applications for practitioners and patients, reducing paperwork and processing time. The new online submission process means you can save your patient a trip to a Service NSW Centre to hand in a form.

What is the Mobility Parking Scheme (MPS)?

The Mobility Parking Scheme (MPS) provides parking concessions to support people with disability or mobility impairment to access the community and participate in everyday activities. Permits must be displayed when using these concessions and can be issued to eligible individuals or to eligible organisations transporting a person with a disability.

Eligibility is based on medical criteria and applies to individuals who are unable to walk, have significant difficulty walking distances, or require the use of mobility aids such as crutches, walking frames, scooters, or wheelchairs, or are permanently blind.
There are three types of permits available:

– Individual permits (blue card) –issued for longer-term or permanent conditions;
– Temporary permits (red card) –issued for short-term conditions; and
– Organisation permits (green card) –issued to organisations that transport people with disability.

Some applicants who hold a driver or rider licence may also need to undergo a Fitness to Drive medical assessment to support safe driving outcomes.

Where do I submit a medical certificate for a Mobility Parking Scheme application?

You can submit medical certificates electronically via HealthLink SmartForms from within your Practice Management System (PMS) or via the MyHealthLink Portal. This allows for real-time submission and faster processing.

Is a Fitness to Drive assessment always required?

No. A Fitness to Drive assessment is not required for all applicants. However, applicants who hold a driver or rider licence may be required to undergo a Fitness to Drive medical assessment if it is their first application for a Mobility Parking Scheme (MPS) permit, they have developed a new medical condition that may affect their fitness to drive, or they are renewing an MPS permit and have not undergone a medical assessment within the previous six months.

What does the new online Mobility Parking Scheme medical certificate mean for patients?

The new online Mobility Parking Scheme medical certificate SmartForm will:

  • Make it easier and more time-efficient for applicants to apply for, renew or replace a permit, reducing the need to visit a Service NSW Centre, saving travel and waiting time for patients
  • Digitally and securely capture and store medical information, maintaining patient confidentiality.
  • Digital application guides will guide/assist each application through the process improving the digital experience and minimising application errors.
  • Automatically verify an applicant’s patient’s identity if already established (for example, from a driver licence or Photo Card). If a applicant’s patient’s identity cannot be verified, then they will be prompted to visit a Service NSW Centre.

Currently, the application for an MPS permit is paper-based and is submitted to Service NSW in person or via an online portal after printing out, scanning and uploading the completed form.
Service NSW will introduce a new digital pathway for customers to apply, renew or replace a permit, alongside existing support channels.

What will not change for the Mobility Parking Scheme (MPS)?

  • Supported access channels will continue, including Service NSW centres and phone support.
  • Eligibility criteria.
  • Medical certification to determine eligibility.
  • Photo requirements.
  • A physical permit will still be issued.
  • Permit fees and concession arrangements
Which patients can’t complete a Mobility Parking Scheme (MPS) application online?

While requesting the permit online will be more convenient for most patients, some will need to attend a Service NSW Centre.

Patients will need to attend to a Service Centre for any type of permit request if:

  • They need the permit immediately
  • They are requesting the permit on someone else’s behalf
  • They are under 16
  • They hold a dual permit
  • They currently hold a temporary permit and need to get an individual (5-year) permit
  • They do not hold a NSW Driver Licence or Photo Card.

Additionally, patients will need to attend a Service Centre for their new permit or their permit renewal if:

  • They do not have a photo on file with Transport for NSW, or the photo is too old, and they do not have a photo exemption
  • They currently hold an individual (5-year) permit and need to get a temporary permit.

Finally, patients will need to attend a Service Centre for their replacement permit if they are replacing the permit because:

  • A change of name or appearance
  • They did not receive their permit in the mail.
