The Australian Government’s My Aged Care service is the entry point for older Australians to access government-funded aged care. My Aged Care e-Referrals, powered by HealthLink SmartForm technology, were introduced in 2019 to provide fast, convenient and secure referral to My Aged Care.
Working with the Department of Health, Disability and Ageing, updates were made to the My Aged Care e-Referral SmartForm aligned to the introduction of the Support at Home program and the new Aged Care 2024 Act. These changes came into effect on 1 November 2025 and include:
- the introduction of the Support at Home Program;
- the Support at Home End-of-Life Pathway;
- Younger People in Residential Aged Care – YPIRAC; and
- the introduction of Aboriginal and Torres Strait Islander Assessment Organisations.
